Want to find a special way to let this awesome person know what she means to you? Here’s how: write up in a few sentences or paragraphs your feelings on this topic:

*or Step-Mom/Grandmother/Sister/Team Mom or other “Motherly Figure”
Include the following:
- Your name
- Grad year
- Club team
- City/State
- Name and relation to the who you’re honoring
- Photo of you and this person (optional, but preferred)
- 1-4 sentences or paragraphs on why your mom or mom figure is important to you… why you love her, how she helps, supports or teaches you or how she just makes you feel special! Feel free to include a story or example as well!
